If you want to transfer to BOAZ from your current provider of medical cannabis, please read this!

You have a choice:

Before October 17, 2018, transferring medical access to cannabis was difficult and required action by your physician. Starting on October 17, 2018, legislation has allowed the transfer of medical access documents between Licensed Medical Cannabis Producers, giving medical clients greater freedom to look for cannabis products, pricing and customer service that are a good fit.

If you are interested in transferring to BOAZ as your new Licensed Medical Cannabis Producer, please review this website or call us and we can answer any questions you may have. If you have decided to go forward with BOAZ, we can begin the transfer process with two documents that you can download, complete and submit to us: Medical Document Transfer Request Form and Medical Client Registration Application Form.

Once we have received your forms, we request your original medical document from your current Licensed Medical Cannabis Producer. After receiving and reviewing your medical document alongside the details you submit to us in your forms, we will register you as a client of BOAZ to purchase medical cannabis, or if the Cannabis Regulations require that we not register you, we will return your Medical Document with a notice that we cannot register you. In circumstances where the Cannabis Regulations require that we not return your Medical Document, it will not be returned to you.

If you are cultivating cannabis plants under your medical access, we need one more document – a copy of your Registration Certificate, which is a document that Health Canada would have sent you with your name, address and date of birth, your health care practitioner, your daily quantity of dried cannabis, and a unique registration number. If you are not sure to which document we are referring, please contact us. Please keep your original Registration Certificate as received from Health Canada.
